It’s insurance hell day.  Still battling 2 year old bills.  Just another part of cancer or being sick.  Navigating the bills and the insurance.  Third parties often bill for physicians that aren’t part of a hospital (independent contractors).  Bills come very far after the services were rendered.  Since bills come very late often times over years, the insurance companies try to place that bill in a year where your deductible isn’t met (deductible shopping).  However, they must apply the bill on the year of the date of service or when the first claim was received.  Normally our deductibles are met the first month of every year.  If bills are too confusing, which they want, you can hire a billing advocate to assist with navigating this very convoluted system.  You have the right to all your itemized bills.  Many time there are basic data entry errors preventing the claim from being processed and sometimes it is up to the patient to correct the errors.  Insane!!
